Customer Service and Marketing skills required

Registered Nurse with current licensure in NYS

LEAD participants into full participation in ALL educational and recreational aspects of the program.

Ensure that all documentation and other correspondence from all medical appointments, Court visits, etc. is delivered to the case manager.

Add variations and supplemental information that will stimulate and expand the skills of the participants.

Implement the lesson plans (extra learning program) and life skills groups provided by the program coordinator

Submit all necessary documentation to Case Manager, Program Coordinator, HR Dept., and Executive Director as requested.

Excellent communication and organizational skills

2 year’s current experience in specialty area

Other certifications may be required for certain specialties

Location: Evanston Hospital, Infant Special Care Unit

Independently provides safe, effective, and compassionate nursing care for all patient populations in the ISCU unit.

Utilizes the nursing process in planning, implementing, and evaluating the patient plan of care.

Includes patient and family in the plan of care as appropriate.

Exceptional organizational and time-management skills, with the ability to handle multiple tasks effectively.

Conduct training sessions and workshops to continuously improve the skills and knowledge of our caregivers.

High school diploma or equivalent; additional education or certification in healthcare a plus

Maintain accurate records of staff documents, certifications, licenses, and training credentials.

Collaborate with our team to create customized care plans tailored to each client's specific requirements.

Proven experience in a coordinator or administrative role, preferably in home care or healthcare.

Coordinates review and utilization of Care Management reports

Oversees the daily operation of the HCH

Provides leadership in the implementation and coordination of Healthcare Home activities

Designs and develops health and wellness initiatives

Champions Healthcare Home practice transformation

Assists development of staff training on health issues

Writing lively and compelling summaries, headlines and captions in a manner that is consistent with Times style

Handling fast-moving breaking news presentations

Building and/or executing special presentations for the home screen

Workshopping and sending push notifications

Will include some nights and weekends

Minimum 2 years Experience in QI Role and/or Management Experience in Home Health

Provide Training to Improve Patient Outcomes and the Patient Experience

Provide Continuing Education to All Disciplines

Exceptional Oral and Written Skills

Minimum 5 years Experience in Home Health
