Registered Nurse- Triage

The Triage Registered Nurse for CareBridge is a key member of the acute care clinical team and is responsible for providing triage and disease education through telehealth modalities to patients who receive Home and Community Based Services (HCBS) services through state Medicaid programs. The Triage Registered Nurse works closely with the family and natural supports, paid caregivers, CareBridge providers and primary care physicians to ensure the patient receives the necessary care to keep them healthy and safe at home.

Responsibilities:

• Reinforce cost-effective, high-quality care to patients.
• Maintain excellent punctuality and attendance during work hours.
• Participate in clinical case conferences.
• Other duties as assigned.
• Perform follow up and check in visits with patients as needed during acute illness.
• Provide compassionate care to diverse patients and their families.
• Triage the care of individuals with a multitude of health problems with urgent care issues.
• Recognize and respond promptly and appropriately to emergency situations.
• Provide effective patient education using the Teach-Back technique.
• Document all patient encounters per documentation standards.
• Thrive as a member of the interdisciplinary team and facilitate the continuum of care.

Qualifications:

• Holds active, unencumbered, compact RN license
• Working knowledge of computers and ability to document effectively and efficiently in an electronic system
• Expert communicator over the telephone, providing timely, appropriate advice and/or guidance with health care issues
• Experience in care of pediatric and adult, chronically ill patients
