This position is responsible for timely and effective management of patient calls requiring medical triage and coordinating care during the transition of care following an inpatient medical stay. Provides guidance, education and facilitates resources for the management of the at-risk population.
This is a virtual position, serving a region of the health system; currently residing locally in Colorado. Although the position is primarily remote, the job will require on-site meetings and trainings occasionally in Denver, Colorado.

Unit/Location: Triage and Transition Registered Nurse is a part of the SCL Health (now Intermountain Health) Medical Group in Colorado.

Minimum Qualifications:

• ASN / ADN from an accredited program with the ability to obtain your BSN from an accredited program within four (4) years of hire unless you possess a minimum of fifteen (15) years of proven continuous Registered Nurse experience in an acute care setting, required
• Current Colorado RN license or compact license with the ability to obtain Colorado RN license, required.
• Basic Life Support (BLS) Certification for healthcare providers.
• One (1) year related experience in a clinic or acute hospital nursing setting is required.

Preferred Qualifications:

• Bachelor's Degree in Nursing (BSN). Education must be obtained from an accredited institution.
• Resident of the local Denver, CO area

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.

$30.97 - $55.02
