Job Description
$2,000 Sign-on Bonus!
Pay Range: $19.00 and onwards depending on experience.
RADIAS Health is seeking a Team Coordinator to support the FACT team members and clients served on the team. In addition to answering phone calls, typing, filing, and scheduling, this person performs duties such as financial record keeping, coordinating of conferences, obtaining office supplies, coordinating direct mailings, and working on special projects. The Team Coordinator interacts with a diverse group of external as well as internal contacts at all levels of the organization. Independant judgement and critical thinking skills are required for planning, prioritizing, organizing a diversified workload, and recommending changes in office practices or procedures.
Hours: Monday - Friday 8:00 am - 4:00 pm
This position works onsite at: 166 4th St E, Saint Paul, MN 55101
Duties And Responsibilities
  • Inform and consult with staff for the proper management of clinical documentation, court paperwork and insurance verifications.
  • Operate and coordinate the computer and management information systems for the program.
  • Promptly complete scanning and uploading into the client electronic chart.
  • Prepare requests for medical records and client authorizations and maintain log of requests and records received.
  • Collect necessary data and prepare quarterly, biannual, yearly and output/outcome reports as required for county and state.
  • Review client charts regularly for completeness and accuracy.
  • Design and maintain forms and spreadsheets.
  • Actively participate in team meeting in collaboration with the team leader.
  • Sort, distribute and prepare mail and faxes to staff and/or upload to client chart.
  • Prepare team documentation reports and maintain all informational databases regularly.
  • Coordinate ordering and maintain supplies and equipment with supervisor approval.
  • Assist team lead with managing scheduling of clients’ appointments and treatment plan meetings with team and prescriber.
  • Facilitate organization and clear communication of team needs.
  • Track client data and update spreadsheets and electronic healthcare record regularly.
  • Schedule and coordinate client transportation needs.
  • Answer telephone and attend to client and staff issues in a timely and courteous manner.
  • Participate as a member of the Safety Committee.
Requirements
  • Familiar with Microsoft Windows and Office programs.
  • Knowledge of office management with good clerical and organizational skills.
  • High school diploma or equivalent.
  • Experience with electronic health record preferred.
  • Excellent clerical and organizational skills.
