Job Description
Provides telephonic triage and direct patient care after normal business hours. Follows care plan, provides treatments, and works collaboratively with the members of the team to help meet positive patient care outcomes. Directs and supervises LVN's as necessary.
  • Provides accurate and timely documentation of patient services to reflect the plan of care, including submission to the of office.
  • Participates in coordination of hospice services, appropriately reporting the identified needs for hospice interdisciplinary group members (CHHA, OT, PT, MSW, Dietitian, Manager, ST).
  • Assist with the continuity of care/changes in LOC (GIP/Respite/Routine SNF admissions) and discharge planning from facilities as required.
  • Provides direct patient care and telephonic triage, as defined in State Nurse Practice Act.
  • Ensures medications profiled with HP in a timely manner and DME ordered as appropriate.
  • Provides appropriate pain/symptom management. Evaluates patient's response to treatments/medications.
  • Responds to urgent and family/patients calls during "after normal business hours" through collaboration with on-call contracted answering service.
  • Performs other duties as assigned within scope of practice.
  • Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self plan of care.
  • Directs and supervises LVN's to respond to on-call needs and patient visits.

  • Understanding of guidelines and scope of nursing practice.
  • BLS required within 60 days of hire.
  • Current drivers license and proof of insurance required.
  • Current California RN license required.
  • Minimum of one year clinical nursing experience required. Previous Home health or hospice experience required.
  • Able to work independently with minimal supervision to ensure that patient care is adequate and appropriate.
  • Able to work as part of interdisciplinary team to effectively manage patient care.
  • Able to effectively communicate with all team members and customers.
