- Reviews tracking and reporting of RN driven visits
- Performs and documents basic patient history, treatments, and administered medications appropriately in EMR.
- Plans for long and short-term provisions of supplies and equipment to meet patient needs.
- Supports RN development activities through active participation with nursing staff in the identification of learning needs during new employee orientation, training and continuing education.
- Cooperates with other health care personnel to coordinate patient care and establish mutual goals.
- Performs other duties as assigned.
- Assigns lunches and lunchtime coverage to RN staff.
- Reviews patients records and other pertinent information. Takes appropriate action as required.
- Triages emergency and non-emergency phone calls providing appropriate direction. Documents in EMR according to PCC established protocols.
- Assists and coordinates with interdisciplinary team leads to maintain patient flow.
- Delegates work assignments as needed (i.e. nurse visits).
- Functions as a team lead as assigned. Team Lead duties include:
- A ssesses walk-in patients as well as other RN driven visits.
- Maintains patient confidentiality.
- Develops work schedules, monitors daily staffing needs and assignments.
- Performs laboratory and diagnostic tests as ordered by the provider.
- Monitors all RN team desk tops. Monitors work load of the Team Nurse, assists Team Nurse with duties as needed.
- Monitors and evaluates patient care by observing and guiding staff performance according to clinic standards of care and practice, policies and procedures.
- Initiates and pursues own plan for continued professional growth.
- Assists providers with treatments and procedures as directed.
- Accurately documents in EMR and reports to the provider any pertinent information regarding patients condition and care.
- Notifies patient by telephone, certified mail, or via PCC patient portal of any abnormal test results as directed by provider.
- Maintains nurse driven protocols and standing medical orders in collaboration with leadership
- Drive continual quality improvement through participation in organizational quality improvement efforts that may include but are not limited to patient satisfaction, clinical and operational quality improvement initiatives.
- Maintains a neat and orderly clinic work area.
- Assists clinical staff with the orientation of new RN staff. Assesses educational needs of clinical staff, provides additional training as needed.
- Functions as a resource for all clinical and non-clinical support staff.
- Responds to physician and patient complaints in collaboration with site leadership
- Assesses the patients condition and utilizes teach back method to provide necessary patient education.
- Directs patient flow. Assists other staff in need of assistance, to maintain patient flow.
- Complies with PCC customer service guidelines and protocols.
- Complies with PCC policies, procedures, and safety practices.
- Administers prescribed medications and treatments as directed by the provider following PCC policies and procedures.
- Coordinates and evaluates daily operations to ensure the delivery of quality patient care and the effective and efficient management of the clinic under the direction of the Clinic Administrator.
- Obtains vital signs and performs routine functions to prepare patients for examinations or procedures.
- Keeps providers informed of changes in assignments or patient flow that will affect working conditions.
- Graduate of an accredited school of nursing.
- 2 3 years clinical experience preferred.
- Constantly communicates with patients, families, and other healthcare providers. Must be able to exchange accurate information in these situations.
- 2 - 3 years EMR experience preferred.
- Current and valid State of Illinois nurses license.
- Current and valid CPR card.
- Must be able to move around the clinic site 50% of the time.
- Must be able to cover other shifts as necessary.
- Ability to move as needed to perform necessary nursing skills such as wound care, injections, and other procedures.
- Occasionally positions self to maintain exertion of physical strength to move objects of 10 pounds from one level to another.
- Constantly operates a computer, computer printer, copy machine, and telephone.
- Must be able to transport from one site to another.
- Must be able to remain in a stationary position 50% of the time.
- Ability to read and write proficiently using the English language.
