Position Summary The Referral Nurse for the Gregory Bunker Care Transitions Center ("Bunker") is responsible for evaluating and determining patient eligibility for recuperative care and medical respite programs. The Nurse will accept referrals from providers, managed care plans, and hospital systems and evaluate patients' physical and mental health needs and ability to thrive in a communal setting. The Referral Nurse must possess strong customer service and communication skills. Required Qualifications * Current license as a Registered Nurse with the State of California * Two plus years related experience preferred or an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job. * Current First Aid and CPR certifications * Knowledge of acute and chronic medical conditions including wound care, mental health and substance use disorders. * Knowledge of community resources including Medicare, Medi-Cal, and Managed Care Plans. * Knowledge of medications, usages, interactions and effects. * Ability to review hospital medical records, assess and evaluate patients' ability to thrive in medical respite facility, and record findings. * Provide care coordination and transition with partners. * Ability to react calmly and effectively in an emergency situation. * Ability to maintain detailed records. * Ability to establish and maintain positive professional relationships with team and partners * Strong organizational skills to support a fast-paced environment. * Good computer skills, including word processing and e-mail preferred. * Good oral and written communication skills. Essential Responsibilities * Work with patients experiencing homelessness and co-occurring disorders such as mental health, chronic medical conditions, and substance use disorders at a high acuity care facility requiring 24/7 patient support and monitoring. * Accept intake calls and faxes from referring hospitals and providers. * Evaluate referrals for admission while ensuring the safety of patients and the community. * Assess referred patient's health status and discuss any health concerns. * Collaborate with partnering agencies and or patient caretakers as needed for coordination of care purposes. * Complete referral intake within 24-48 hours (holidays excluded) hours or notify the supervisor that intake has not been completed. * Coordinate care with supporting agencies such as home health nurses, physical therapy, etc. * Collaborate with Recuperative Care case managers regarding patient care needs. * Provide medical education regarding disease management and nursing care as necessary to both patients and staff. * Notify the partner of the client's status (acceptance or denial). * Coordinate medical appointments and make follow-up calls. * Make referrals to local agencies such as Guest House for psychological evaluations. * Collect information for patient files. * Assign beds. * Notify staff of incoming clients. * Participates and supports in-service to partnering hospitals as needed. * Support the Director in monthly briefings and or updates to partnering hospitals. * Assist in the coordination of visiting physician assessments and or client collaborations. * Adherence to infection control procedures, including but not limited to, standard precautions of temperature monitoring, hand washing, symptom self-monitoring, masking and social distancing. * Assist and support the Center/Department/Program to meet standards of High Reliability. * Other duties as assigned.
