Charlie Health has set out on a mission to reimagine how high acuity care is delivered to young people and families in crisis. Our initial offering is a virtual intensive outpatient program, which places peers with similar mental health experiences and goals into customized virtual groups. Our team of masters-level clinicians lead groups multiple times per week to deliver a higher level of care.

About The Role


Every month, thousands of people call Charlie Health in search of care for themselves or a loved one. While we strive to serve as many people as possible, not everyone is appropriate for treatment at Charlie Health. For example, someone might need services that we don’t currently provide. These people still need somewhere to go, and it’s the Referral Coordinator’s responsibility to help.


The Referral Coordinator will have the opportunity to work directly with patients and families to identify alternative treatment options. To do this, the Referral Coordinator will search Charlie Health’s national provider database. Then, together with the family, they will select an appropriate resource and make the referral via phone, fax, and/or web.


The Referral Coordinator will also help build and maintain Charlie Health’s relationships with external providers. This person will be expected to communicate professionally and collaboratively with external providers, identify areas where additional referral options are needed, and make updates to the database as provider information changes.

Responsibilities


  • Adhere to stated policies and procedures and achieve performance metrics goals
  • Work directly with clients, families, and referral sources to understand their needs and preferences
  • Work closely with the Clinical Outreach and Partnerships teams to build a deep understanding of referral sources and the services they provide
  • Make accurate and timely outbound referrals for who are not admitted to Charlie Health
  • Function as a liaison between Charlie Health and partners to ensure all ongoing needs are met and the client experience remains at the center
  • Collaborate closely with internal stakeholders at Charlie Health (e.g., clinical team, admissions team) as needed to fulfill job responsibilities
  • Make referrals using external provider’s preferred communication channels
  • Follow-up with clients and/or external providers to verify placement
  • Use and maintain a nationwide provider database to identify appropriate referral options
  • Ensure a supportive, positive experience for clients and referral sources / external providers
  • Document all client and referral source interactions in the electronic record system


Qualifications


  • Extreme organization and attention to detail
  • Strong interpersonal, relationship-building and listening skills
  • Metrics- and results-oriented mindset, with experience working against concrete targets
  • Excellent written and verbal communication skills
  • Work authorized in the United States and native or bilingual English proficiency
  • Ability to thrive in a fast-paced environment and learn quickly
  • Minimum of 2 years of relevant work experience (e.g., experience in healthcare, preferably in customer / patient-facing roles such as case management, discharge planning, referral relations, admissions, or outreach)
  • Proficient in Salesforce and Google Suite/MS Office
  • Bachelor’s degree in health sciences, communications, psychology, social work, or related field


Additional Information


The expected base pay for this role will be between $60,000 and $70,000 per year at the commencement of employment. However, base pay will be determined on an individualized basis and will be impacted by location and years of experience. Further, base pay is only part of the total compensation package, which, depending on the position, may also include incentive compensation, discretionary bonuses, other short and long-term incentive packages, and other Charlie Health-sponsored benefits.
