- Performs other job-related duties as required.
- Conducts work essential to supporting outreach efforts, including but not limited to:
- Collecting and verifying compliance data
- Entering data
- Summarizing findings
- Recording the results of outreach
- Reporting results to management
- Outreaches patients to schedule appointments. This includes determining what barriers or obstacles exist that prohibit patient from making or keeping appointment and determine what resources may be needed or action that should be taken.
- Collecting and verifying compliance data
- Reads, understands and applies HEDIS/HCC specification and/or policy as it relates to compliance and outreach.
- Educates providers and other staff on quality related activities (e.g. HEDIS, HCC)
- Summarizing findings
- Recording the results of outreach
- Reporting results to management
- Reviews patient’s charts as needed to determine compliance based on specification or process outlined. Determine next steps based on chart review findings.
- Entering data
- Determines possible root cause to issue effecting patient compliance and presents issue and possible solution(s) to management
- Commitment to enhancing patient experience required
- Experience with HEDIS preferred
- A minimum of two years’ experience in quality related activities preferred
- A minimum of two years’ experience in health care setting required
- High School diploma
- Must have computer proficiency, with proficient use of Microsoft Word, Excel, and PowerPoint required
- Knowledge of HEDIS quality measures required
- Excellent interpersonal, communication, organization and writing skills with the ability to effectively prioritize work required
- Experience with PCMH preferred
- Bachelor’s Degree preferred
- Knowledge of utilizing electronic medical records systems (such as EPIC) preferred
- Experience with HCC preferred
- Ability to adjust quickly to change required
