Job Description
We are seeking a dynamic and compassionate individual to join our team as a Patient Admissions Coordinator. As the first point of contact for patients and their representatives, you will play a crucial role in providing exceptional customer service and ensuring a smooth admissions process. Responsibilities
Serve as the primary liaison between referral sources, clinics, and discharge planners, utilizing your exceptional sales and customer service skills to place patients effectively.
Manage case management tasks, ensuring timely coordination of chair times, financial clearance, and medical clearance for an expedited admissions process.
Be the initial contact for patient referral calls, displaying excellent phone etiquette and the ability to multitask effectively.
Coordinate the entire admissions process, from the initial request for admission to patient placement in their preferred clinic, adhering to established procedures and regulatory requirements.
Collect, complete, and communicate all necessary intake data for admission requests, collaborating with clinic and management staff as needed.
Demonstrate resilience and the ability to handle objections and pushback, ensuring necessary information is obtained for patient placement.
Provide support to other teams during peak periods, assisting in the processing of referrals and calls when required.
Conduct interviews with patients, their representatives, or referral sources to gather personal and financial data necessary for determining eligibility.
Utilize call tracking and admissions program systems to accurately document calls, with familiarity in Salesforce and EMR systems being a plus.
Maintain and update Salesforce with current data, relevant information, and the status of admissions referrals, demonstrating excellent communication and documentation skills.
Complete preliminary paperwork and standard admissions forms to ensure efficient processing of admissions.
Collaborate with multiple departments and resources within our organization to successfully place patients across our operational footprint.
Promote the efficiency of the admissions process by providing professional and timely assistance to callers, handling escalations when necessary, and collaborating with supervisors and managers.
Work closely with insurance verification teams to ensure patients are financially cleared for admission to our facility.
Communicate promptly with individual clinics, scheduling patients and forwarding necessary documents and records in a timely manner.
Collaborate with facility staff and management to resolve any issues that may impact a patient's admissions process.
Immediately report any admissions denials from clinics or billing groups to supervisors and managers for further action and communication.
