Job Description
Job purpose


The Patient Engagement and Outreach Coordinator is responsible for internal and external outreach initiatives and activities to promote AACI health center services to community residents who lack access to health care and health insurance. This position is directly responsible for conducting patient engagement activities with the goal of increasing patient enrollment for AACI’s health centers.


Duties And Responsibilities


  • Collaborate with local organizations to build awareness of health insurance coverage options, spur enrollment, and build referral linkages.
  • Participate in media events.
  • Other related duties and projects as assigned.
  • Assist with the development of public education and information materials, presentations and displays and distribute outreach materials to patients, community members, partner organizations and businesses.
  • Develop and implement a community outreach plan to promote the services provided by the organization.
  • Engage prospective patients and enroll eligible patients into appropriate insurance plans.
  • Identify and establish relationships with community influencers (e.g.: Promotoras) who can increase AACI’s visibility in the community and help drive patient enrollment.
  • Develop training program for community and health worker outreach.
  • Recruit, train and utilize volunteers to increase program capacity.
  • Attend community events to promote coverage options and services of the organization.


Qualifications


  • Bilingual ability (Spanish or Vietnamese) an asset.
  • Available to attend evening and weekend community events and meetings on a regular basis.
  • Ability to establish and maintain effective work relationships as part of a multi-disciplinary team .
  • Ability to perform the duties described above.
  • Experience facilitating public meetings and other public forums.
  • Ability to communicate well with people of diverse cultural professional and experiential background
  • High level of written and presentation communication skills.
  • High School diploma or equivalent.
  • Knowledge of health care coverage plans such as Medi-Cal, Covered California, and PCAP (Santa Clara County) preferred.
  • Three or more years of experience in enrollment navigation, community outreach, community relations or a related field.
  • Ability to possess and maintain a valid CA Driver’s License, must have suitable vehicle for agency travel with current automobile insurance.


Working Conditions


Work is partially performed an office environment. The majority of work time will be spent performing in-field outreach and enrollment assistance activities. Extensive interaction with the public is required. Driving is essential to the function of this job. Regular evenings and weekend work is required.
