The Patient Care Coordinator I is a member of a dynamic team focused on coordinating all aspects of patient care. The role provides continuity of care by ensuring a smooth transition between care settings and by developing a trusted relationship with patients to identify and assist with each patient’s individual needs that may arise related to the care they receive.
Responsibilities:
- Collects and enters patient demographic and insurance information and enter into registration databases.
- Assists with the financial clearance process as necessary.
- Receives and responds to patient feedback.
- Recognizes emergent or urgent requests and initiates appropriate pathways for clinical support.
- Notifies or escalates access challenges to appropriate parties as needed.
- Responds to other patient requests as appropriate.
- Works closely with patients, families, visitors, providers, and staff to identify needs, answer questions, resolve problems, provide information and follow up.
- Communicates with providers, techs, and nurses regarding the patient’s scheduling needs.
- Schedules and coordinates patients’ appointments.
- Manages and prioritizes scheduling requests.
- Triage calls from referring physicians, patients, family members, and others wishing to access care at FHCC.
- Active listening skills
- High school graduate or equivalent
- Ability to identify and triage issues appropriately
- Ability to work independently, organizing, and prioritizing concurrent responsibilities and demands
- Strong computer skills and familiarity with MS Office
- Ability to demonstrate sensitivity and empathy in an emotionally challenging environment
- Ability to communicate verbally in person, on the phone, and in writing in a clear and concise manner
- Ability to work in a high pressure, time-sensitive, and complex health care environment
- Attention to detail to ensure scheduling accuracy
- Ability to build relationships while maintaining appropriate boundaries.
- One year of customer service experience required
- Coping skills
- Previous medical office/hospital experience coordinating physician appointments and patient services preferred
- Prior experience utilizing an electronic medical record system and/or an automated scheduling system
- Familiarity with medical terminology
- EPIC and/or RIS, or any electronic medical records system
- Bilingual skills
- Bachelor's degree (BA or BS)
- Prior experience in a clinical or healthcare setting
