Description
Arms Acres is looking for a Case Manager to join our outpatient clinic in Carmel, NY. In this position the Case Manager assumes full charge of patient case management, delivering individual and group mental health and chemical dependency services including assessment, discharge planning, managed care, and relevant communication, formulation and implementation of treatment plans, and performing specified duties.
Requirements

• CASAC-T or CASAC Certified Required
• Associates Degree in Chemical Dependency or related field with 1 year of applicable experience.
• CPR Certification required within 6 months of hire

SIGN ON BONUS ELIGIBLE for QHPs: Final amount dependent on Full Time Equivalent Status for selected shift. Maximum amount is $3,000 to be paid at three intervals (90 Days, 6 months and 1 year)
