The Clinical Coordinator RN at Loretto Health and Rehab (700 E. Brighton Ave) is responsible for coordinating the clinical practice of staff and following up on areas of concern to assure holistic care is provided to each resident. Serves as liaison between resident, family, physician, and Loretto staff. Provides daily assessment of each resident and communicates changing needs to appropriate personnel. Serves as a liaison between clinic staff and reports to a Director of Site Operations, Program Director or Nurse Manager.


  • Completes own tasks promptly and ensures staff completes tasks in a timely manner.
  • Assists in data gathering for QA/QI reporting as required; assists in evaluation of QA/QI trends and participates in plan of correction development, implementation, and compliance rechecks.
  • Orients and trains new clinical staff, ensuring all clinical and regulatory requirements are met and the orientation and competency paperwork has been completed.
  • May work with a scheduler to ensure patients are scheduled/rescheduled for appointments and tests as necessary.
  • In working with the team, ensures the unit is in compliance with NY State Department of Health regulations and in compliance with corporate and regulatory policies and procedures; addresses issues appropriately.
  • Ensures environment is clean and safe for patients by conducting rounds. Initiates corrective actions as required. Assists in the monitoring of compliance with QA checks (refrigerator, code cart, etc.) and works with the nurse manager to ensure policy adherence.
  • On a daily basis, reviews all required documentation for completion and accuracy.
  • Facilitates communication of vital information to all staff as necessary; reviews changes/adjustments to schedule.
  • Acts as primary liaison for team and contracted providers; provides direct input into patients’ care plans as appropriate, communicates changes for treatments/medication to interdisciplinary team and contracted providers and care givers as appropriate.
  • Functions as a staff RN when necessary. May support the Director of Site Operations, Program Director or Nurse Manager in providing adequate coverage of nursing personnel for vacations, absences, holidays, etc.
  • Monitors follow through on provider’s verbal and written orders to ensure completion including lab work, consults, appointment scheduling, rescheduling and follow up etc.
  • Directly oversees nursing care provided to patients or participants in a department, function or site. Assigns and directs staff’s daily work and all day to day activities. Follows up to ensure duties are carried out.
  • Provides health training to participants, family and caregivers as required; assesses participant abilities and suggests interventions to maximize independence and quality of life.
  • Leads or participates in staff in-service and meetings; fulfills in-service requirements and oversees compliance of department, function or site staff in this regard.
  • Identifies systems/processes in need of improvement and makes recommendations to Director of Site Operations, Program Director or Nurse Manager.
  • Monitors medication and other patient related errors. Takes action to minimize occurrence and follows up appropriately per policy and/or regulations.
  • Monitors and ensures the proper use of equipment in assigned area. Provides instruction or obtains maintenance and repairs as needed.

  • Ability to manage multiple priorities in a busy and sometimes noisy environment.
  • 1+ years supervisory experience.
  • Current New York State RN license and registration in good standing required.
  • Proven patient assessment and clinical skills.
  • Actively leads or participates in meetings, teams and task forces as requested.
  • Manages and supervises assigned personnel, including performance management, scheduling, and orientation. With the Director and Human Resources guidance, makes or approves recommendations on employee hires, transfers, promotions, salary changes, disciplinary actions, terminations and similar actions. In conjunction with HR, resolves grievances and other personnel problems in a fair, timely and consistent manner.
  • 3+ years nursing experience preferably in long term/acute care.
  • Ability to communicate effectively with clients, residents, and families from diverse backgrounds who may have physical, sensory and mental impairments.
  • Proficient in computer programs and able to learn new software rapidly.
  • Uses effective communication and critical thinking skills during conflict resolution, problem solving, decision making, and other complex dialogues.
  • Promotes and encourages teamwork among employees and supervisors, and between departments. Communicates ideas and goals clearly and is an effective listener. Ensures that information is effectively communicated and shared throughout the department and organization. Provides and encourages effective coaching. Promotes a positive environment of constructive feedback.
  • Uses effective communication skills, such as, active listening, unbiased and nonjudgmental language, and open communication style with all staff.
  • Delegates, organizes and coordinates the daily operations of departments within the function, department or site.
  • Ability to analyze problems, plan resolution strategies, implement solutions and evaluate outcomes.
  • Knowledge of NY DOH regulations.
  • Current knowledge of best practice nursing standards and the care planning process.
  • Proven organizational and time management skills.
  • Fosters process improvements for greater operating efficiency and resident/customer service. Ensures optimal workflow and adequate resources.
  • Ensures adequate skills, training and development among staff.
  • Graduate of an accredited school of nursing. BSN or BS in related field preferred.
  • May have input to or recommendations for operating budgets.
  • Ability to manage and guide staff while carrying out the duties of an RN.
