Responsibilities:
The Oncology Nurse Navigator (ONN) is responsible for managing the care of identified patients with cancer, in collaboration with a multidisciplinary team, to ensure access to high quality care throughout the continuum.

PRIMARY DUTIES AND RESPONSIBILITIES
  • Assesses and promotes healthy lifestyle choices and self-care strategies through education and referrals to ancillary services.
  • Assesses health literacy and potential barriers to learning, and incorporates the Oncology Navigation Standardized Teach Back Health Literacy script when educating and communicating information to patients.
  • Applies insight and understanding concerning human emotional response to assess and provide emotional and psychosocial support, and facilitates appropriate referrals for patients, families and caregivers especially during periods of high emotional stress and anxiety.
  • Assesses educational needs of patients, families and caregivers and facilitates individualized care within the context of functional status, cultural considerations, health literacy, psychosocial, reproductive/fertility and spiritual needs.
  • Assesses oncology patients in the seven domains of care in the NHCI Standardized Oncology Navigation Supportive Care Survey upon initial encounter and periodically through the cancer care continuum using motivational interviewing and OARS communication techniques and covering areas on the Supportive Care Survey Reference Card.
Qualifications:
REQUIRED:
- Master's Degree in Nursing and two (2) years of clinical experience in oncology. - OR Bachelor of Science Degree in Nursing and three (3) years of clinical experience in oncology. - OR Associate Degree in Nursing with five (5) years of clinical experience in oncology.
PREFERRED:
- Previous experience working with culturally diverse populations. - Strong working knowledge of healthcare systems and community resources. - Able to establish strong working relationships within a healthcare system. - Masters of Science Degree in Nursing. - OCN certification.
