Job Description

SUMMARY

Registered Nurse - The Agency (InCommunity) Registered Nurse performs nursing activities, supports, and care to individuals in accordance with the policies of Georgia Community Support and Solutions, Inc (GCSS) and enable of Georgia DBA InCommunity and the State of Georgia Nursing Licensing Bureau.

ESSENTIAL DUTIES AND RESPONSIBILITIES includes the following:

Serves as the overall supervisor of nursing within nursing.

Contacts doctors as needed to report, clarify, or discuss healthcare concerns, including individuals’ change in health status.

Reviews all admission referrals for appropriateness of services.

Conducts assessments and evaluations as needed.

Provides monitoring for all individuals receiving support and services from InCommunity.

Identify and set up educational/training needs of staff.

Provide consultation and direction to the support services team regarding the health concerns of individuals served and advise agency management on the development of internal protocols that support the best healthcare outcomes.

Perform monthly status checks on AED units, and quarterly checks on medical equipment and devices in the group homes.

Provides consultation regarding critical incidents (Image Report).

Maintains highly organized, consistent, thorough, and systematic recordkeeping practices.

Collaborates with Health and Regulatory Manager and Wellness Coordinators to monitor the health and safety of individuals enrolled in services.

Collaborates with departmental staff regarding doctor’s orders, Medication Administration Records (MARs) to ensure they are completed correctly.

Develop Care Plans, complete HRST reviews as needed for all individuals in case load to ensure adequate support to meet needs of individuals with high medical acuity.

Ensure monitoring of medications for individuals receiving services, assist staff with medication management issues.

Provides nursing care and assessments communicating with departmental staff and obtaining reports for status updates on hospitalized individuals with prompt nursing assessment following hospital discharge.

Serve on CQI committee to ensure that areas that need improvements are identified and in collaboration with other members of the nursing staff, develop and implement measures to promote improvement. Assist in the completion of CQI report and other reports as needed.

Perform performance evaluations for staff directly under the positions.

Attend staff meetings, provide health related education training to staff to meet individual’s needs and provide input for Individualized Service Plan (ISP)

Collaborate with department staff on a routine basis to discuss and resolve issues and to ensure homes are managed in accordance with all applicable roles and regulations.

Other duties as assigned.

QUALIFICATIONS:

To perform this job successfully, an individual must have acute hearing and speech skills to communicate with the public, family members, and outside agency program and management staff. Must have physical stamina to sustain occasional intense periods of activity and stress. Must be able to lift 50 pounds or more.

EDUCATION and/or EXPERIENCE:

Graduated from an accredited School of Nursing and experience in the area of developmental disabilities, preferably with the focus being in a community based programs. Must have 2 years’ experience intermediate computer skills. Experience in effectively evaluating the compliance of the program. Knowledge of family systems and structures.

CERTIFICATES, LICENSES, REGISTRATIONS:

Current Georgia RN License.

PHYSICAL DEMANDS:

While performing the duties of this job the employee is frequently required to sit. The employee is occasionally required to stand; walk; use hands to finger, handle, or feel; reach with hands and arms; climb or balance; stoop; kneel, crouch, crawl, talk or hear; and taste or smell. The employee must frequently lift and/or move up to 25 pounds and occasionally lift and/or move up to 50 pounds.

Physically able to occasionally kneel bend/stop, squat, push/pull, reach above and below shoulder level frequently. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.

COMPUTER SKILLS:

To perform this job successfully, an individual should have knowledge of basic popular software; advanced Microsoft Excel Spreadsheet software and advanced Microsoft Word -Word Processing software. Basic knowledge of software systems.
