The Opportunity:

As a Hospice Aide for our Hospice House, you will have the opportunity to provide patient care directly in the comfort of their own home. You will work closely with patients and their families and be a key piece in the end of life care we provide to our patients. You will assist with daily living activities and basic home care. There is a lot of autonomy in this position, so you will need to be confident in your skills and enjoy working independently.

Shift: Full-time nights, 3 12-hr shifts per week

We are looking for experienced CNAs with experience with home health, hospice, long term care or inpatient care to join this growing team. This is a collaborative group that are passionate about their work, they are dependable, reliable and support each other through the care plan process. They help each other learn and grow. The Work:

• Collaborate with other health care team members to enhance the provision of patient care.
• Provide care to patients that are on service with Saint Luke's Hospice.
• Provide care to the patients in their home.
• Assists in the patient care anticipating patient family needs in a proactive fashion.
