Henry Ford Home Health Care (HFHHC) Nurse Residency Program is designed to assist the novice nurse transitioning from entry-level advanced beginner to competent professional nurse. The program will be facilitated by nurse educators and supervisors engaged in the progressive development of critical thinking and clinical reasoning skills. The RN Residency program supports the novice nurse in the delivery of safe, quality care within a collaborative inter-professional healthcare team environment. Additional support will be provided to the nurse residents through additional clinical workshops. Under general supervision of the nursing supervisor and education staff, utilizes the knowledge and application of nursing principles to provide direct patient care in the home. The nurse resident interprets the philosophy and goals of HFHHC to others in the work setting; functions within established guidelines and demonstrates the ability to work effectively and develop therapeutic relationships with patients, their families and other professional and supportive personnel who provide patient care. Nurse residents will also participate in finding solutions for variances from patient goals and engage in implementation of solutions promoting optimum patient outcomes.
PRINCIPAL DUTIES AND RESPONSIBILITIES
Building upon knowledge and skills acquired in pre-licensure programs, the nurse resident performs patient care processes according to adult and geriatric patient care standards under the close supervision of the nurse educators and nursing leadership. Upon licensure, after 4-6 months of experience, the nurse resident may progress to function more independently.
The nurse resident may assist with or perform (upon licensure) any and all of the following duties:
- Responsible for the assignment/supervision of licensed practical nurse and home health aide services provided in the patient’s home.
- Responsible for increasing the patient’s awareness of medical condition and teaches the prevention of illness and maintenance of general health.
- Reports and records significant information and changes to the physician in charge, with regard to the patient’s condition, and maintains accurate and timely clinical records to be included in the patient’s medical records.
- Participates in in-service programs, discharge planning and professional committees.
- In conjunction with the attending physician, re-evaluates the patient’s outcomes for care, their continued needs, the effectiveness of services rendered and plans for discharge on a regular basis, altering health care plans accordingly.
- Provides counseling to patients and their families to meet patient goals and nursing needs.
- Conducts a comprehensive physical assessment, including the Outcome and Assessment Information Set (OASIS), as required, by obtaining vital signs, monitoring lung sounds, assessing and treating wounds, and reviewing laboratory tests related to problem or illness.

- Provides direct patient care to patients in accordance with physicians’ orders. Identifies factors affecting health care by evaluating the patient’s physical, mental, emotional, environmental and social needs with regard to clinical diagnoses and mental condition.
- Administers medications and treatments as prescribed by a physician and monitors the patient’s response.
Adheres to all divisional, corporate, regulatory, and HIPAA requirements related to the privacy and security of confidential patient information.
Follows established priorities to meet deadlines.
Promotes and ensures Home Health Care's commitment to the Henry Ford Experience.
Supports and complies with the HFHS Team Member Standards of Excellence.
Maintains a safe work environment and practices personal safety techniques to prevent injury to self and others.
Complies with corporate and divisional policies and procedures.
Performs other related duties and projects as assigned.
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED
- BLS Heath care provider certification preferred. Successfully completes within orientation.
- Successfully passes NCLEX-RN and holds an active, unrestricted license within 90 days of hire. Failure to pass NCLEX-RN within 90 days will result in termination of the Nurse Resident position.
- Sign a statement of commitment.
- Graduation from an accredited school of nursing with 6 months or less of experience in a related area of nursing practice.
Strong interpersonal and customer service skills required in order to interact effectively with individuals from various cultural and socio-economic backgrounds. Must possess a current State of Michigan driver's license and a satisfactory driving record.
Must have reliable transportation.

WORKING CONDITIONS
Works in the patient’s home, which is an uncontrolled environment.
May be exposed to potentially infectious materials, blood borne-disease pathogens and hazardous waste.
May need to lift, carry, bend, stoop or otherwise be physically active in the course of job duties.
Drives to and from the patients' homes located in the Detroit Metropolitan area in varied weather and traffic conditions.
