Nurse Practitioner- Triage
Summary:The Remote desktop Nurse Practitioner or Physician Assistant is responsible for utilizing his/her patient care skills by using desktop computer skills to deliver primary health care to patients. The APN will handle prescription refills, lab reviews, referrals, and telephone or computer messages from patients who are seeking medical information and/or medical attention. They work as a team which includes MDs, APNs, and the onsite support teams members.
Essential Duties: Assess and evaluate and respond to the portal message or telephone message sent. This includes reviewing the patient’s health care record in regard to the patient’s illness or request, as well as advising the patient in home self-care when appropriate and facilitating an office visit when necessary. Responsible for deciding and coordinating the appropriate care/ disposition for the message which may include test results or test requests, medication refills, or internal and external referrals.
Responsibilities:
MDT Navigator NPs, by utilizing their critical thinking skills, educational background and experience, will provide appropriate patient care. Will work effectively and efficiently to:
·Provide continuity and consistency for the patient’s care.
·Collect and analyze critical patient information.
·Electronically document the information by utilizing the Epic EMR.
·Provide a documented permanent addition to the patient's medical record.
·Collaborate with other team members when appropriate to ensure the patient's safety and well-being.
·Approve prescriptions and referrals.
·Consistently work in a positive and cooperative manner with fellow MDT Team Members, as well with the on-site care team (MDs, NPs, RNs, MAs, Patient Service Representatives)
- Other duties as assigned.
Skills & Knowledge:
·Medical knowledge and skills applicable to perform care up to and within the scope of practice.
·Thorough knowledge of medical terminology, anatomy, and physiology
·Critical thinking skills as it relates to primary and internal medicine care.
·Communication skills (verbal, written and listening skills) with the ability to work effectively within the organization and especially with patients requiring education, consultations, and advisement.
·Technical and analytical skills for use in the analysis and interpretation of tests, procedures, and results
·Good knowledge of computer applications, such as Microsoft Office Suite (Word and Outlook), Electronic Health Record (EPIC or similar).
·Ability to work independently, as well as be part of the team, including accomplishing multiple tasks in an environment with interruptions.
·Ensure the privacy of each patient’s protected health information (PHI).
Requirements:
- Minimum Three years of practical NP experience.
- NPI
- MDTriage operates in Pacific Standard Time.
- Current CA NP or PA license
- EMR proficient, Epic preferred.
- Graduate of an accredited school of nursing required.
- Computer proficient.
- Reliable high-speed internet connection for provided VOIP phone (DSL or Cable must have a minimum of 5 Mbps upload).
This is a unique role and the candidate must be willing to work Monday - Friday throughout the weekday. Part-time and Full-Time opportunities exist.
