This Neuroscience Nurse Navigator will be responsible for:
- Advocating for patients to promote patient centered care that includes shared decision making and patient goals of care with improved access and patient experience outcomes.
- Guiding patients through the health care system and assisting to overcome barriers to care needs.
- Initiating communication with patients using various methodologies including patient portal, phone and mail
- Clinically triaging new patient referrals efficiently, to ensure timely access to care.
- Educating patients and families regarding disease management and medications.
- Coordinating care with specialty and primary care practices to maximize efficient scheduling and clinical oversight for enhancement of the right services needed and matched with the right providers.
- Assisting with completion of prescriptions, school forms , medication and DME insurance authorizations, and other community networking resources and referrals
- Contacting patients for changes in clinical management.
- Providing oversight, delegation and act as a resource to unlicensed staff, including clinical assistants and patient experience representatives.
- Working with departmental physician, nursing and administrative leadership to optimize current systems and create new systems for effective and efficient new patient triage
- Providing ongoing care coordination between clinic visits by communicating with families, healthcare team members and other agencies involved.
- Assessing, planning, implementing, evaluating and modifying plans of care for a specific patient assignment throughout the healthcare continuum.
- Bridging care across the continuum, including new patient calls and transition of care across care settings (inpatient to outpatient, ED to outpatient) and the community.
- Working 40 hours per week: either four 10 hour shifts or five 8 hour shifts Monday through Friday
To qualify, you must have:
- Strong telephone and technology navigation skills.
- At least 5 years of pediatric nursing experience in either inpatient or ambulatory setting, and at least 2 years of neurology and/or epilepsy experience preferred.
- Knowledge of pediatric complex, chronic and family centered care.
- BSN degree and CPN, CNRN or AMB-BC professional nursing certification.
