Job Description
The CM- DI works under direct supervision of the Manager, Disease Intervention, the CM-DI in this position provides direct disease and intervention awareness to the public, as well as education, counseling and referral services to individuals that are at risk of STD/HIV infections.


Essential Duties and Responsibilities:


  • Performs Phlebotomy activities as directed to help identify individuals that are at risk STD/HIV infection.
  • Requires a professional phlebotomist to travel at least 1-2 hours outside of Coconut Creek and surrounding areas.
  • Conduct client registration in the EMR and complete Quest Lab requisitions.
  • Requires knowledge of varied social, economic, cultural, and lifestyle characteristics and the ability to communicate with clients as well as target groups of diverse individuals.
  • Participate in community screening events by performing phlebotomy (specimen collection) to help identify individuals at risk of STD/HIV. These events can occur before, during or after normal working hours including weekends or holidays.
  • Perform blood collection/Phlebotomy and Urine collection
  • Travel to different site/location to conduct STD/HIV Screenings
  • Completing the IMG Case Management Form, 1628, and Hepatitis C Risk Assessment Form.
  • Perform related duties as assigned by the Supervisor.


Requirements:


  • Excellent communication and interpersonal skills
  • Highly organized and a proven strategic thinker with ability to engage and motivate teams
  • Solid knowledge of EMR/EHR systems and telehealth.
  • Strong knowledge of PrEP (Pre-exposure prophylaxis)
  • Perform Specimen collection/ Blood and Urine for STD/HIV Testing
  • 1 years of extensive working knowledge of STD’s/HIV
  • Strong analytical and problem-solving skills
  • 1 year of Disease Intervention or related field and/or equivalent professional experience.
  • 1 years in Case Management (Preferred)
  • 500/501 HIV Training
  • Strong work ethic; excellent interpersonal and influential skills
  • Hepatitis Testing Training
