Job Description
Job Summary:


The SHERO Intake Coordinator will supervise the Intake Specialist. The Intake Coordinator will be responsible for ensuring that all referrals have been screened and referred to the SHERO Subcontractors. The Intake Coordinator will monitor the flow of referrals, admissions and discharges. The Intake Coordinator will report to Rikers Island at minimum two times per week to conduct intake screenings.


Responsibilities:


  • Review and screen all SHERO referrals
  • Participate in individual supervisory sessions and case reviews with the Program Director, and other staff as required
  • Responsible for data collection on referrals, admissions and discharges
  • Coordinate the availability of beds among the SHERO consortium
  • Attend program meetings, agency-wide meetings and staff training
  • Oversee the Intake specialist schedule and duties, and provide direct supervision
  • Collecting any/all documentation required, provide general information, explain house rules, expectation of behavior, and emergency procedures to referring partners
  • Supervise Intake Specialist
  • Maintain ongoing contact with ATI, public defenders and other community agencies concerning referrals for potential residents’, and participate in regular Referring Partner meetings with providers as scheduled
  • Assist with any and all data collection systems
  • Manage pending referrals list
  • Report to Rikers Island to conduct interviews and screenings
  • Performs other duties as assigned by Program Director
  • Responsible for assessing the needs of the referred resident and submit appropriate referrals to subcontractors to ensure individual needs are provided by community partners
  • Maintain appropriate and up-to-date documentation of services for all clients; keeping thorough and accurate records, files, and correspondence in written and electronic form.
  • Responsible for updating SHERO occupancy report and admission and discharge data log.


Requirements


  • Excellent interpersonal and organizational skills
  • Meaningful understanding and commitment to housing first, harm reduction and trauma-informed care
  • Excellent verbal and written communication skills and the ability to work independently
  • Excellent judgement and ability to handle crises effectively
  • Experience with conflict resolution and mediation preferred
  • Experience facilitating groups highly preferred
  • 4 years of case work experience, preferably working with women with homeless histories, behavioral health needs, and/or criminal legal history. Preferable work experience with families and children
  • People directly impacted by the criminal legal system highly encouraged to apply
  • Master’s Degree.
  • Competent technology & management skills, including proficiency with using Microsoft Office, and client database management systems
  • Bilingual in English and Spanish strongly preferred
  • High level of self-awareness and ability to give and receive feedback well
