We are currently looking to hire an Registered Nurse to join us as a Hospice Intake Coordinator, based at our office in West Orange, NJ. The position would be responsible for providing assessment of patients’ hospice needs and coordination of care for hospice patients.


Responsibilities


  • Adheres to the organization’s policy in regards to absenteeism and appearance
  • Establishes and maintains professional working relationship with hospital staff, assisted living facility staff, skilled nursing facility staff, physicians, and VNAHG staff
  • Provides individualized counseling to patient and family regarding hospice services
  • Omission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position
  • Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
  • Ensures compliance with regulatory and statutory requirements
  • Coordinates referrals to Hospice to promote continuity of care


Qualifications


  • Excellent interpersonal and organizational skills
  • 2+ years off hospice nursing experience
  • Valid NJ Registered Nurse license
  • Strong client focus, assertive, decision making skills, articulate, and creative in problem solving
  • Excellent verbal and written communication skills
  • Bachelor’s degree in Nursing preferred
  • Strong decision making skills


Working Conditions/Physical Demand : Fast paced health care environment with phone, scanner, email, fine motor hand movement and computer use. Use of personal automobile with valid insurance and driver’s license.
