Bloom Healthcare is looking to add an Intake Coordinator to our team! This position will be responsible for handling new patients from the time of first contact until the first appointment with a provider. We’re looking for passionate individuals committed to improving the lives of homebound seniors. The position will report to our Director of Operations, and collaborates with our administrative team, providers, intake and our community care partners.


Intake Coordinator Responsibilities:


  • Building patient charts in the EMR
  • Entering med lists
  • Scheduling new patient appointments
  • Following up with referral source
  • Tracking new patients and referrals
  • Verifying insurance
  • Communicating with patients and those involved with the patients care about the practice
  • Mapping patient visits


Intake Coordinator Qualifications:


  • Experience in healthcare preferred
  • 2 years of customer service experience in an office setting
  • Bachelor's degree preferred
  • High school diploma
