Endoscopy Nurse Navigator - Full Time, Days

This position will also have responsibilities to travel to Orland Park and Hinsdale. In this role you will partner with the rest of the clinical team to provide patients the highest quality of care.

Essential Job Functions:

• Works with clinical program manager to prepare offsite procedural data monthly to present to faculty or administration as needed
• Ensures all external referrals are received for specified payors patient pre-appointments
• Ensures timely procedures for patients requiring urgent diagnostic or therapeutic procedures
• Works collaboratively with providers, referring providers and offices, scheduling team and insurance to expedite appointments for patients based on clinical need and priority
• Follows all FIT positive test results to ensure that Primary Care MD has been notified and developed a plan of care
• Is educated on the various insurance plans UCMC accepts and can recommend patients to their respective insurer if appropriate
• Assists endoscopists in post-procedure follow-up care
• Point of contact for referring providers from within UCM (internal) and outside (external)
• Reviews value based performance reports and routinely recommends interventions if targets are not on course.
• Collaborate with GI Physicians for plan of care for cases that are complicated (transportation, education, additional resources needed etc.)
• Systematically reviews all scheduled patients for GI colonoscopy and endoscopy at all sites of care to ensure patient’s insurance and clinical risk factors are aligned with the requirements of the scheduled site of service (HOPD, ASC, POS-11).
• Demonstrates ability to educate patients and families. Educates patients on colorectal screening tests, including colonoscopy vs. annual FIT
• Reviews scheduled patients for GI colonoscopy and endoscopy to ensure all required documentation is present (lab, path, records/charts, referrals) and ensures patient is educated on colonoscopy PREP process, referring back to PCP if necessary
• Maximizes endoscopy access by monitoring daily schedule and filling available appointments, as well as prioritizing care based on patients' clinical needs
• Reviews offsite provider schedules for all scheduled colonoscopies and endoscopies and routinely recommends patients to scheduling team to move up to ensure provider schedules have no gaps
• Oversees and facilitates scheduling of all procedures requiring anesthesia

Required Qualifications:

• Excellent interpersonal skills required, with specific ability to communicate effectively with patients, family members, and physicians.
• Minimum of two years clinical nursing experience with demonstrated clinical competence in patient care, teaching and management of patients.
• Registered Nurse with a current Illinois license.
• Must have general computer skills and be adaptable to changing working situations and work assignments.

• Gastrointestinal (GI) care experience
• Certified Nurse in Specialty area preferred or in process.
