Job Description
Position Summary
Responsible for assisting in the planning, developing, organizing, implementing, evaluating, and directing of Transportation Programs in accordance with current existing federal, state, local and DOT standards, as well as our established policies and procedures, to ensure the medical, emotional, recreational, and social needs of the resident are maintained on an individual basis.
Qualifications/Experience/Requirements
  • Any combination of experience and training which provides the required skills, knowledge and abilities.
  • Prefer training in first aid and CPR.
  • Must possess violation free MVR report.
  • Must possess, as a minimum, a high school diploma
  • (1) year CDL driving experience preferred.
  • Certified Nursing Assistant Training preferred
Essential Functions
  • Driver will use caution while transporting residents to avoid motions that could be detrimental. A red cross first aid and CPR training course is desirable.
  • Transport residents for medical/dental appointments as scheduled by Medical/Transportation Receptionist.
  • Meet with Director, on a regular basis, to assist in identifying and correcting problem areas, and/or the improvement of services.
  • Keep chart on all incidents, mileage, gas used and number of residents transported daily. A weekly report is to be submitted to the Director in addition to a month-end full report.
  • Safety check list to be completed before transporting residents. Vehicle safety equipment check list must be done weekly. Both checklists are to be turned into the Director.
  • Transport residents for shopping, banking and social outings as scheduled. Monitor residents' locations and safety during outings. Load and unload groceries, packages for residents after grocery shopping. Must have awareness of Social calendar and events scheduled at community.
  • Be alert for any change in resident's condition. Report to Director of Nursing immediately.
  • Driver to assist and advise residents of safety measures loading on and off vehicles. Driver to wear safety belt while loading residents on and off vehicles.
  • Receptionist must be advised when drivers leave and return to the facility with residents. A log is to be completed on residents who board the bus. A copy is to be left with the receptionist the original to be kept on the bus assuring all residents who left are returned.
  • In the event of an emergency (i.e. fall, heart attack) do not remove resident, call 911 for medical assistance. Upon returning to community notify immediately the nurse on duty and Director.
  • Recommend to the Director the equipment and supply needed.
  • Before maintenance is performed on vehicles-the vehicle maintenance request form is to be completed and turned into the Director to receive prior approval. All invoices to be submitted to the Director.
  • Report all unsafe/hazardous conditions/equipment immediately.
  • Keep abreast of current federal, state and DOT regulations, as well as professional standards, and make recommendations on changes in policies and procedures to the Social Director.
  • Maintain CONFIDENTIALITY of all pertinent resident care information to ensure resident rights are protected.
  • Maintain an excellent working relationship with the medical profession and other health related facilities organization.
  • Check g·as, oil, antifreeze, brake fluids, platform and tire pressure on a timely basis to maintain the vehicle in good running order. Vehicles to be kept clean and orderly at all times. Keep vehicles free of loose objects which could fall or roll.
  • Participate in the overall quality assessment and improvement programs.
  • Ensure that all established safety regulations are followed at all times.
  • Assist in maintaining a good working rapport with other departments within the community, and outside community health, welfare and social agencies, to ensure that programs can be properly maintained to meet the needs of the residents.
  • Assist in planning, developing, organizing, implementing and evaluating the transportation programs of this community.
  • Emergency numbers must be kept in all vehicles at all times.
  • Report all incidents/accidents immediately.
  • Coordinate with the-Director or Resident Services to assist residents with transportation in compliance with medical and/or leisure needs. Communicates with Director or Resident Services for medical appointments to set up times residents should be ready to leave for their medical appointments.
