Community Living Coordinator


Reports To: Director of Programs & Services


Hours of Work: Hourly, nonexempt, 38 hours per week - hours may vary as needed

POSITION SUMMARY: Lead institutional transition and diversion efforts. Provide independent living skills training to individuals who have disabilities. Provide peer counseling and support to consumers on caseload. Conduct presentations on relevant topics related to independent and community living. Conduct and participate in individual and systemic advocacy activities.


EXPECTED RESULTS: Serve a minimum of 65 active consumers per year. Achieve a program consumer goal accomplishment rate of at least 65% Assist 20 consumers per year avoid institutionalization or return to the community from institutions. Assist 60% of those served each year increase their ability to access transportation, housing, health care and/or adaptive equipment. Assist 70% of those served each year increase their ability to care for themselves, their family or household. Ensure 70% of consumer served will remain independent in their homes.


ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.


Service Delivery


  • Participate in individual and systems advocacy
  • Provide individual peer support to caseload
  • Provide independent living skills training
  • Provide information and referral services
  • Identify individuals in institutions who want to return to the community
  • Track and monitor consumer progress
  • Conduct initial assessment to determine consumer’s individual needs
  • Assist consumer in developing initial, intermediate and long range goals
  • Provide and coordinate services designed to help individuals in institution return to the community, and help individuals remain in the community


Public Relations/Marketing


  • Provide community organizations with training related to disability issues
  • Conduct public education and awareness activities


Administrative/Clerical


  • Track community activities and information and referral contacts
  • Gather program outcomes based evaluation data
  • Activate, track and inactivate consumer files using the League’s policies & procedures
  • Maintain current and accurate consumer files, case notes and database entries


Financial


  • Monitor and adhere to established program budgets, including grant requirements attributed to the program.


Planning Incumbent is not directly responsible for any League planning activities.


Personnel Incumbent is not directly responsible for any League personnel activities.


SUPERVISORY RESPONSIBILITIES: Incumbent is not directly responsible for any League supervisory activities.


QUALIFICATIONS, REQUIREMENTS and DEMANDS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skills, and/or abilities required. EDUCATION and/or EXPERIENCE: Bachelor’s degree (B.A.) from a four-year college or university; or two years of post-secondary education and two years of related experience and/or training; or an equivalent combination of education and experience. Prefer a qualified person with a disability.


LANGUAGE SKILLS: Ability to read, analyze, and interpret common professional journals, legislation/regulations and legal documents. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or other community entities. Ability to write routine reports and correspondence. Ability to write routine reports and correspondence. Ability to effectively present information to top management, public groups, and/or community groups.


MATHEMATICAL SKILLS: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers common fractions and decimals. Ability to calculate figures and amounts such as percentages and ratios.


REASONING ABILITY: Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions and regulations, and deal with several abstract and concrete variables.


INTERPERSONAL SKILLS: Ability to work cooperatively, effectively and efficiently with volunteers, agency board members, staff members, agency management and the general public. Ability to quickly build positive relationships with volunteers, agency board members, staff members, and visitors/callers. Ability to work independently, maintain positive working relationships, and function consistently as a contributing member of the League’s staff and management team.


PHYSICAL DEMANDS: While performing the essential functions of this of this job the employee is required to: 1. regularly talk and hear to communicate, 2. regularly use fingers, hands, and arms to manipulate objects, 2. regularly sit, stand, and walk and, 3. occasionally lift and/or move up to 20 pounds.


EQUIPMENT OPERATION REQUIREMENTS: Ability to independently operate, or learn to operate TTY and voice phones. Ability to independently operate, or learn to independently operate League office equipment such as fax/copy machines in order to perform job related duties and assignments. Ability to independently operate, or learn to independently operate the League’s computer network and software in order to perform job related duties and assignments.


TRAVEL REQUIREMENTS: Ability to travel independently: 1. frequently throughout the League’s service region and, 2. occasionally in an interstate region. Must have dependable transportation to meet these requirements.


WORK ENVIRONMENT: While performing the duties of this job, the employee is frequently exposed to outside weather conditions. The noise level in the work environment is usually moderate.


WORK SCHEDULE DEMANDS: Must be available for work during regularly scheduled business hours. Must be available evenings and weekends as work demands require.
