Position Summary
Responsible for providing clinical triage to practices supported by the NYP Sphere contact center. Works collaboratively with the triage nurse manager, contact center Patient Financial Advisors, Contact center leadership, practice(s) leadership, practice(s) clinical and non-clinical teams, and the CCTC team.
Essential Job Duties
General Duties:
- Conducts assessment over the phone utilizing appropriate clinical triage protocols and obtains pertinent information relative to the patient’s health and the situation presented by the patient or caregiver at the time of the call.
- Manages triage needs that are identified when patients are seeking appointments with their providers by working collaboratively with patients to best understand their needs and aiming to reduce readmissions, enhance chronic disease management, and manage any health risks for patients where applicable.
- Documents timely assessments and recommendations in patient’s medical record.
- Provides therapeutic advice to all specified patients
- Functions as a centralized clinical team member along with the patient’s practice clinical team for designated patients in a variety of specialties and primary care by assisting with the coordination of care in accordance with the patient’s outlined plan of care.
- Participates in training and on-boarding activities along with any relevant or ongoing process improvement initiatives, professional development, peer reviews, and performs other duties and responsibilities, as assigned.
- Performs other job-related duties as assigned.
- Books appropriate appointment for patient evaluation based on the level of urgency and in accordance with triage recommendations.
