What you do:


The Clinical Liaison builds and maintains professional relationships with discharge planners and other relevant hospital or physician staff, ensures that relevant hospital and physician relationships know and understand services and products provided by Mission Point. Assesses patient needs through chart review and where necessary direct patient contact. Meets with patients and families to explain and educate regarding Mission Point services and products.


  • Meets with patients and families to explain and educate regarding Mission Point services and products.
  • Ensures that relevant hospital and physician relationships know and understand services and products provided by Mission Point. 
  • Builds and maintains professional relationships with discharge planners and other relevant hospital or physician staff.
  • Coordinates availability issues with Mission Point sources and seeks necessary approvals to reach a timely patient decision.
  • Secures required level of care determination information and completes and distributes intake paperwork.
  • Assesses patient needs through chart review and direct patient contact.
  • Assists Admission Directors in expanding Mission Points patient level of care awareness network and more.
  • Has a working knowledge of the hospital and its various departments regarding what departments generate patient awareness about the medical specialties and services provided at Mission Point and the sub-acute level of care. 


What you need:


  • Registered Nurse (RN), Licensed Practical Nurse (LPN), Social Worker (SW) or other appropriate clinical degree. Graduate from accredited college / university with a current license, as appropriate, to practice in the State of Michigan and in good standing. 
  • Experience in management preferred.
  • At least two years clinical experience required.
