The Intake Coordinator must be a Licensed Practical Nurse (LPN). The employee coordinates the intake of patient referrals to the Agency, communicating with Agency staff, and facilitating timely admission of the patient to home health services.

Qualifications:


  • Must be currently licensed LPN in the state in which practicing
  • Ability to exercise independent judgement and multitask, ensuring the completion of many tasks, projects, and reports throughout the day.
  • Basic functional knowledge of Microsoft suite applications, ability to navigate websites, and learn an electronic medical record system effectively.
  • Experience with Medicare home health including, but not limited to Medicare home health admission requirements preferred. An understanding of medical terminology and basic coding required.
  • Able deal effectively with high levels of stress and communicate effectively in those situations.
