Plan, implement, and maintain spiritual services to meet patient, family/caregiver needs both during the patient's illness and for 12 months following the death and to function as a source for spiritual, grief and bereavement issues for the community and hospice service area.
Duties and Responsibilities Spiritual Counseling Functions
  • Work with staff, clergy and community groups to enhance their sensitivity to the spiritual concerns of patients/families experiencing terminal illness and loss.
  • Participate in patient conference by exploring and assessing the potential spiritual needs of patients/families and reporting on services as indicated.
  • Maintains confidentiality of all records and information.
  • Provide direct spiritual support and/or counsel to patients/families in keeping with patients'/families' beliefs.
  • Perform occasional liturgical assignments such as the Love Lite Tree Service and other services as deemed necessary.
  • Maintain proper records of visits to patients/families.
  • Provides consultation to interdisciplinary team members about spiritual counseling.
  • Provide bereavement follow-up services as defined by HOM procedures.
  • Develops and recommends spiritual programs and services, as needed.
  • Make contact with clergy or appropriate representatives or patients/families as indicated.
  • Demonstrates professionalism and dependability.
  • Possesses problem solving skills, is self-directed and capable of working with little supervision.
  • Provide educational programs for community clergy, religious and lay representatives as resources allow.
  • Adheres to Hospice Satilla code of ethics and mission statement.
  • Develop and maintain a resource group of clergy to who specific aspects of spiritual care may be delegated.
  • Conduct or make arrangements for funeral or memorial services when indicated
Delegation of Authority
  • Delegated the authority, responsibility, and accountability necessary for carrying out your assigned duties.
Educational/Qualifications
  • Shall possess, as a minimum, an appropriate certification in hospital or pastoral ministry. A Bachelors Degree in theology, social work, psychology, or related field is preferable.
  • Demonstrated experience in working with patients/families dealing with life threatening illness and death.
  • Demonstrated ability to be open, sensitive, flexible and ecumenical.
Experience
  • Minimum three (3) years of spiritual counseling or ministerial work.
