Care Manager Jobs

CDAC is seeking a Care Manager to provide case management services to women and families with substance use concerns in the Okaloosa/Walton Counties. The position requires clinical skills and experience with substance use and mental health disorders, parenting and family issues, and community resources. Candidate must demonstrate an understanding of case management and care coordination principles and have experience working with people with substance use and mental health concerns and must demonstrate an understanding of ROSC (recovery-oriented systems of care) principles. Position available serving Okaloosa/Walton Counties with occasional service to other counties in Circuit 1. Bachelor's degree in social services field with at least two years of relevant experience required. Master's degree preferred.
