Who we’re looking for:

Our Care Coordinator will be responsible for coordinating medical, mental health, and addiction services for Forge Health prospective, existing, disengaged, and discharged clients. This person will help clients access and effectively utilize clinical and social services to achieve better health care outcomes. Using an integrated care model, the Care Coordinator will outreach, assess, create care plans, and coordinate referrals for clients who may have complex medical, behavioral, and social needs.


What you’ll be doing:

  • Outreach clients post-program completion to ensure all their treatment and referral needs have been met to sustain overall health and wellness.
  • Identify medical, behavioral, and social needs of each client to develop a client-centered care plan with goals, objectives, and interventions.
  • Identify and secure services designed to meet the goals and objectives of a client’s care plan including, but not limited to assistance in locating a needed provider, assisting with scheduling an appointment, and assistance in securing transportation.
  • Follow up with hospital/ER upon notification of a client’s admission and/or discharge to/from an ER, hospital/residential/rehabilitative setting.
  • Ensure relevant documentation is completed to facilitate efficient referral support including completing and uploading signed assessments, care plans, and consent forms.
  • Psychiatrist, APN, Executive Clinical Director, Clinicians, and external providers that result in Provide advocacy that results in the removal of barriers preventing the client from securing needed services (e.g., working with an MCO to obtain authorization for a cardiology appointment to further assess an undiagnosed heart condition, and securing needed appointment).
  • Follow-up with clients who missed appointments, left against medical advice (AMA), and/or discharged.


How we define success:

  • Bachelor’s Degree in Social Work, Psychology, Human Services, or related behavioral health field; or
  • Strong interpersonal, organizational, writing, and computer skills.
  • Travel for this role is currently reduced, but some field work may be required. Must be able to travel locally when required.
  • Working knowledge of healthcare environments, clinical terminology, and health information systems.
  • Ability to maintain a high level of respect, empathy, and confidentiality for all clients.
  • Associate degree in Social Work, Psychology, Human Services, or related behavioral health field with at least 2 years experience working with mental health and/or substance use population.
