Job Description

Rehabilitation Teacher II
Performs moderately complex (journey-level) rehabilitation planning and instructional work. Work involves identifying residents' habilitation or rehabilitation needs and providing the necessary instruction in self-help or self-care and vocational skills to assist residents in attaining their optimum levels of independent skills and vocational goals. Works under general supervision with moderate latitude for the use of initiative and independent judgment. Provides appropriate training and support for direct care staff and classroom instructors to implement the training programs. Provides effective management and training of staff and ensures training and documentation are occuring.

Essential Job Functions:
Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned.
Develops and implements the Skill Acquistion Plan (SAP) for the individual. Works with the IDT to assure the SAP is functional and practical for the individual. Monitors training is occuring as outlined in their Skill Acquisition Plans (SAPs). Provide directions and support regarding related training activities to the home, instructor and/or substitute instructor. Assure ample daily documentation to provide accurate and timely monitoring of clients' progress toward targeted objectives. Maintains a conducive learning environment to lessen class distractions and enhance clients' opportunities to learn. Attends work on a regularly scheduled basis. Assures the inital and annual assessments are completed and reviewed. Reviews progress monthly and makes comments and remediation as necessary. Inservices staff on individuals' programming and appropriate use of training materials. Ensures client safety by observing training areas for no risks to health or wellbeing.
Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency’s obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.

Knowledge Skills Abilities:
Knowledge of rehabilitation assessment techniques and tools, of the rehabilitative process, of statutory requirements applicable to the rehabilitation process, and of skills and adaptations necessary for employment placement and disabled persons. Ability to assess clients' functional levels; to plan an individualized program of instruction with, or on behalf of, the client; and to communicate effectively. Ability to use computer, especially Microsoft office programs. Ability to work harmoniously with clients and staff. Ability to demonstrate flexibility and willingness to accept additional responsibility if needed. Strength and flexibility to safely perform first aid and/or CPR, Safe Use of Restraint (SUR), and Ukeru techniques.

Registration or Licensure Requirements:
Experience in social and rehabilitation services work or in teaching and counseling persons with disabilities. Graduation from an accredited four-year college or university with major course work in rehabilitation, psychology, sociology, social work, education, or a related field is generally preferred. Experience and education may be substituted for one another.

Additional Information:
Applicants must pass a fingerprint criminal background check, pre-employment drug screen, and registry checks including the Client Abuse/Neglect Reporting System (CANRS), Nurse Aide, Medication Aide and Employee Misconduct and HHS OIG List of Excluded Individuals/Entities. Males between the ages of 18 – 25 must be registered with the Selective Service.
All State Supported Living Center employees are subject to Random drug testing.
Flexibility in work hours may be required for this position. The position may be required to work overtime and/or extended hours. Must be willing to work weekends and holidays.
