Millcroft Living is recruiting for a hospitality focused Admissions Coordinator to join our Health Care Team! This role provides full-scope, hands on admissions functions within the community.
Admissions Essential Job Duties:
1. Obtains referrals from hospital social workers, physicians, or family members keeping wait lists current.
2. Functions as the Health Center public relations person, providing tours of the facility and pertinent information regarding admissions including visits to hospital discharge planners and doctor’s offices.
3. Conducts admission interviews with the resident and/or responsible party, obtaining all required financial data, signatures, pertinent social information and medical information, attending physician information, and other pertinent and required documentation prior to admission.
4. Reviews all admission related paperwork with the resident and or/responsible party including Medicare Part A coverage and non-coverage procedures. Maintains all non medical record related paperwork in an admissions file for each resident.
5. Processes all Medicare denial letters, Medicare certification and re-certifications and according to Medicare regulations.
6. Informs the Medical Records Secretary of on going daily census activity such as admissions, discharges, deaths, room transfers, changes in payment methods, and temporary and permanent apartment status.
7. Maintains Life Care resident statistics, permanent assignments, temporary assignments, and notifies resident and/or the responsible party of permanent and temporary status in the Health Center.
8. Keeps an ongoing daily/monthly census of residents in the Health Center noting admissions, discharges, deaths, transfers, changes in payment methods, and temporary and permanent assignments.
9. Maintains an accurate list of number of days an independent living resident resides in the Health Center.
10. Coordinates utilization review for new admissions covered by Medicare Part A.
11. Maintains complete confidentiality regarding admission information and resident records.
12. Assists in coordinating admissions/transfers/discharges with other departments.
Social Services Essential Job Duties:
1. Identifies the medically related social services and emotional needs of the resident.
2. Provides social services to residents requiring such services or makes referrals to Social Agencies if necessary.
3. Makes arrangements for financial assistance for residents to appropriate agencies.
4. Ensures that the resident, family, and responsible person are fully informed of the resident’s personal and property rights.
5. Records pertinent social data about personal and family problems medically related to the patient’s illness and care, and of action taken to meet the resident’s needs.
6. Maintains records of each resident referral to outside resources.
7. Ensures the confidentiality of all residents’ social and medical information.
8. Serves as the facility’s Discharge Planning Coordinator and executes all responsibilities and functions delineated in the discharge plan.
9. Conducts an initial social service assessment of all admissions, completes the social service sections on the MDS, and maintains current and up-to-date social service progress notes on all residents.
10. Attends all resident interdisciplinary care meetings and coordinates resident and/or responsible party attendance at these meetings.
