Homeless To Housing Case Manager Jobs

Assist program participants in developing vocational, educational and housing goals reflective of their skills, capabilities and interests.

Conduct assessments for barriers and skills such as education level, work history, substance abuse history, disabilities, etc.

Assist program participants in developing vocational goals reflective of their skills, capabilities and interests.

Complete entry in San Diego’s Homeless Management Information System at initial assessment and each re-assessment.

Intensive case management services by conducting and documenting assessments for homelessness vulnerability, housing barriers and financial sustainability.

Manage participants progress towards achieving goals by creating and updating plans.

A minimum of 1 year of case management or counseling experience is preferred.

Ability to discern and report relevant client information to clinical staff for appropriate management of client care.

Bachelors Degree from in a related field is required. Lived experience preferred.

Ability to manage potentially traumatic situations including distressed clients, clients’ family members, and/or responsible parties,

Must possess good verbal and written communication skills.

Knowledge of 42CFR and how it applies to Community & Family Resources.

Provide life skills education and assistance, including budgeting, apartment upkeep, stress management, conflict resolution, etc.

Develop and implement individualized case management services plans that ensure housing stability and identify short and long-term goals.

Provide education to clients on tenant rights and responsibilities, eviction prevention counseling and advocacy.

Provide crisis intervention and ensure that clients have access to the 24/7 case management crisis line.

Experience working with individuals who have a mental illness, chronic health issue, and/or substance use disorder OR

Experience working with individuals affected by the criminal justice system preferred.

Excellent writing skills, including ability to provide quality documentation of case management encounters.

Two years providing case management/social services/outreach including harm reduction and motivational interviewing techniques.

Coordinates closely with benefits advocacy staff to facilitate SSI application process.

Strong organizational, administrative and problem-solving skills, and ability to be flexible and adaptive to change.

Excellent interpersonal, verbal, and written skills and ability to effectively work with people from diverse backgrounds and be culturally sensitive.

Make appropriate use of knowledge/ expertise/ connections of other staff.

Demonstrated experience in managing Housing First Intensive Case management.

Ability to work effectively with other employees, clients, users and management; promote and maintain a team environment.

Familiar with and supports all team members in the use of HMIS (Homeless Management Information Systems data base).

Five years mental/behavioral health direct care experience.

Housing First services experience preferred.

Experience in Oracle HCM desired.

BA/BS in human service field or equivalent combination of education, training, and experience.

Work effectively with other employees, clients, users and management; promote and maintain a team environment.

Maintains up-to-date knowledge of regulations and requirements related to housing programs and client eligibility.

Two years of related work experience in housing programs field or equivalent

Follow through on assignments; accept work assignments from multiple managers and work areas; handle multiple and parallel projects.
