Provides clinical systems remedial support and training as needed or requested by staff or management.

Walk, use hands and fingers, handle or feel, reach with hands and arms, stoop, kneel, crouch. Work at heights from ladders.

Prior care management work experience in a social service agency, physician group, hospital or Emergency Department setting is required

Knowledge, Skills, Abilities and Other Characteristics

Conducts outreach to Case Management Department at hospital when Enrollees are admitted.

Completes screenings, assessments and care plan in accordance with contractual requirements and Care Coordination policy and procedure.

Provides relevant education, counseling and support to assist member with the achievement of goals.

Convenes the Interdisciplinary Care Teams, presents own patients, and provides guidance on others not directly managed.

Collaborates with management to identify training needs and develops training programs.

Certification in Training/Learning & Development required.

Must have minimum three (3) years' experience in a corporate training & design role (onsite and online).

Excellent written and verbal communication skills.

Must be able to pass background check requirements, according to company policy.

Must be tech savvy w/working knowledge of computer and software applications used in job functions

Efficiently manage the scheduling administrative tasks, including creating, updating, and distributing school schedules, data entry, and record keeping.

Proficiently handle scheduling and calendar management for staff, meetings, events, and activities.

Proficient in scheduling and calendar management.

Prior experience in an educational setting is a plus.

Previous experience in administrative or clerical roles is preferred.

Applied Behavioral Analysis (ABA) agency experience preferred, not required

Align WBL opportunities that build upon students' prior experiences

Coordinate and communicate regularly with content endorsed teachers, counselors and industry partners to ensure quality placements and learning experiences

Ensure that students exhibit work readiness expectations of the employer prior to beginning a WBL experience

Implement locally-adopted evaluation to ensure quality WBL experiences for the students(recommended evaluation tools are provided in the WBL implementation guide)

Confer high school credit when WBL course standards are met

Report on activities, placements, and standard attainment

Manage and conduct analysis of work experience activities to ensure implementation of policies and procedures.

Accurately review, enter and complete WEX Timesheets into the DWFS Timesheet Management system.

Track outcomes and manage contractual goals related to the Work Experience Program.

Coordinate a quality work experience program for participants.

Assure data entry is accurate and completed

Communicate effectively with DWFS Corporate staff to follow Corporate WEX review process, resolve paycheck and timesheet issues as needed.
