12 months experience as a licensed nurse in LTC, preferred

Medical Insurance (employer pays 50%)

No weekends, evening or holidays

RN in the State of Minnesota

Serve the St. Cloud/Buffalo area

Drivers License and Clean Driving Record

Excellent interpersonal, communication, and organizational skills.

Ability to manage multiple tasks/projects and deadlines.

Competitive wages with credit for experience

Conduct clinical assessments using root cause analysis.

Interview staff, residents or responsible parties to gather data.

Review medication alignment as part of routine assessments.

Experienced team of Clinical Managers available as resources and mentors

Teach the client and/or family members about cares and support skills

Communicate regularly with RN Case Managers, Supervisors, and the Care Team

Experience with Electronic Medical Records (EMR) software is preferred

Prior Nursing experience in a Home Health, Hospice, or TCU setting is ideal
