3+ years of Clinical Documentation Improvement experience including Chart Review, Provider Querying, and Education

Registered Nurse with 5+ years ICU / ED / Acute Care / Med Surg experience

This is a fully REMOTE, 6-month contract opportunity.

$50-55 per hour depending on experience

Based on review of the existing medical record, determines principal diagnosis, qualifying secondary diagnoses, impacting procedures and assigns appropriate working DRG.

Demonstrates responsibility and accountability for personal development by participating in continuing education offerings.

Interacts regularly with physicians, in the outpatient setting, providing ongoing education regarding compliant documentation.

Maintains HCC documentation requirements, ICD-10-CM code assignments and coding guidelines

Maintains integrity of data collection by ensuring accurate data entry.

Strong knowledge base in coding to include all pertinent coding guidelines

Conducts initial and follow-up concurrent reviews of medical records for assigned patients in the timeframe and frequency defined for productive work.

Utilize advanced knowledge of disease processes, medications, and critical thinking skills to identify gaps in current documentation.

Collaborate with physicians, coding, case management, nursing, and other care givers to facilitate appropriate modifications to documentation.

Certified Documentation Improvement Practitioner Certification (CDIP) or Certified Clinical Documentation Specialist Certification (CCDS)

2 years of inpatient clinical documentation experience

2+ years of medical records review experience

Recent experience working in a metric-driven production role

Review, abstract, and manage the upload of supporting documentation to a third-party platform

Attend and participate in team meetings and educational sessions

Has 3+ years of experience in healthcare or healthcare administration

Has CCDS, CDIP, or CCS certification

Holds CRC certification from AAPC

Lead chart review and abstraction for Medicare Risk patients

Assists in other monitoring activities, special department projects or other needs as determined by the department manager.

Provide ongoing CDI team learning opportunities through sharing of professional knowledge.

Works closely with Operations Manager to implement MRO initiatives.

Knowledge of age-specific needs and the elements of disease processes and related procedures.

Working Knowledge of the Medicare Advantage reimbursement system and HCC Coding required.

Working knowledge of HEDIS quality specification.

Assists in other monitoring activities, special department projects or other needs as determined by the department manager.

Collaborate with clinical faculty and students to ensure completion of clinical orientations and site requirements

Associates Degree or higher and/or extensive experience in healthcare

Excellent communication and presentation skills

Audit and Verification of Student Clinical Documents

Demonstrates knowledge of payor issues, documentation opportunities, clinical documentation requirements, and referral policies and procedures.

Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed.

1-3 years of relevant experience, required.

Facilitates modifications to clinical documentation supporting appropriate reimbursement is received for the level of service rendered to all patients.

Improves the overall quality and completeness of clinical documentation by performing admission/continued stay reviews using clinical documentation (CD) guidelines.

Ensures the accuracy and completeness of clinical information used for measuring and reporting physician and Hospital outcomes.

Conducts educational sessions with physicians and other health care team members on documentation requirements

Conducts CDI on-boarding education of all new admitting physicians as part of the hospitals orientation program

90% of job requires computer data entry and use of software programs.

Physical Abilities & Work Environment

Queries physicians on specificity of procedures performed and diagnosis based on accepted coding guidelines, clinical expertise and LifePoint Hospitals policy

Using clinical knowledge, experience, and problem solving skills to interpret clinical quality data, identify opportunities for improvement, and generate ideas.

At least three years of experience as a registered nurse, preferably in primary care, care management, or outpatient clinics

Prior experience in a quality improvement role or a relevant certification is a plus

Reviewing dashboards of quality metrics, identifying potential issues, and alerting appropriate managers.

Be able to identify and communicate factors that contribute to a good patient experience and outcome.

Reviewing documentation for accuracy, clinical judgment, and adherence to protocols and workflows.
