Clinical Documentation Specialist- Remote ($40.00 - $60.00 / Hour)

Job Responsibilities

• Educate and serve as a resource for physicians and allied health staff to improve the quality and completeness of clinical documentation.
• Identify opportunities in concurrent and retrospective inpatient clinical medical documentation to support quality coding.
• Understand and apply regulatory compliance related to documentation, coding, and billing for all health insurance plans.
• Ensure discharge diagnoses and co-morbidities are accurately documented to reflect patient's clinical status and care.
• Review patient medical records in the inpatient and/or outpatient setting to ensure accurate representation of severity of illness and proper coding.
• Utilize advanced knowledge of disease processes, medications, and critical thinking skills to identify gaps in current documentation.
• Validate coding reflects medical necessity of services and facilitates appropriate coding for accurate reporting of patient's illness severity, risk of mortality, and complexity of care.
• Collaborate with physicians, coding, case management, nursing, and other care givers to facilitate appropriate modifications to documentation.

Required Skills & Qualifications

• Excellent verbal and written communication skills
• Self-motivated and able to work independently
• Strong analytical thinking and problem-solving abilities
• 2+ years of medical records review experience
• Registered Health Information Technician (RHIT), Registered Health Information Administrator (RHIA), Registered Nurse (RN), Registered Respiratory Therapist, Certified Coding Specialist (CCS), or Certified Coding Specialist-Physician-based (CCS-P), or International or Domestic Medical Degree
• 2 years of inpatient clinical documentation experience
• Certified Documentation Improvement Practitioner Certification (CDIP) or Certified Clinical Documentation Specialist Certification (CCDS)
• Recent experience working in a metric-driven production role
