Hospice Medical Social Worker

Develops and maintains thorough knowledge of referral resources throughout the service area.
Provides in-service education and consultation in understanding the social and emotional factors related end of life care.
Master’s Degree in Social Work from Accredited school of Social Work or BSW with 2 years of health care experience.
Collaborates with IDG members, long term care facilities and community agencies to ensure efficient and effective utilization of medical social work.
Provides services in accordance with the mission, vision and values of Suburban Hospice
Completes psychosocial assessments and visit documentation in compliance with agency policy

BSW from an accredited school of social work is minimum education required but must be supervised by MSW.
Participates as a member of the IDT, including development and implementation of the plan of care.
Assists in identifying the need for intervention of other IDT members.
