Social Worker Hospice Home Care Jobs in Iowa

Education and experience related to the care of bereaved individuals

Provides a consistent high quality hospice experience for every patient and family served by soliciting and identifying patient and family goals

Provides assessment, planning, intervention and ongoing evaluation of psychosocial function to meet assigned patient and family needs

Communicates timely and effectively with the appropriate Team Leader/designee, physicians, patients, families, contracted vendors and community resources

In collaboration with Chaplains, provides bereavement support to families

Sensitivity to the particular needs of those in grief

Trained Volunteers- offer companionship for patients and respite for families

Training: Our new staff will feel confident knowing they have learned from experienced preceptors within their area of specialty.

Home care experience as a Certified Nursing Assistant

Experience in a supervised health care setting, such as a hospital or nursing home.

Current Healthcare Provider BLS certification by date of hire

Pay in lieu of benefits premium program

Demonstrated Knowledge, Skills, and Abilities.

Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).

Experience with interdisciplinary health care teams;

Advanced computer skills (i.e.: telehealth)
