Suburban Hospice is seeking a Medical Social Worker that will reflect that mission and ensure effective coordination of social services for patients and their families. We are currently looking for a Medical Social Worker to provide care in Boone, Hendricks and Hamilton counties.
- Collaborates with IDG members, long term care facilities and community agencies to ensure efficient and effective utilization of medical social work.
- Demonstrates organizational skills.
- Remains calm and supportive in crises and other stressful situations
- Develops and maintains thorough knowledge of referral resources throughout the service area.
- Participates in Quality Improvement Activities
- Behavior is consistent with Suburban Hospice mission and values.
- Provides in-service education and consultation in understanding the social and emotional factors related end of life care.
- Communicates effectively with community agencies, team members, patients and families
- Serves as a Community Liaison to develops and maintain positive working relationship with leaders in community resource groups.
- Consistently demonstrates Patient First behaviors.
- Completes Biopsychosocial Assessments.
- Completes psychosocial assessments and visit documentation in compliance with agency policy
- Associate is capable of delivering care/service appropriate to patients/customers
- Maintains accurate and timely records, including clinical and progress notes, according to department policy.
- Provides crisis intervention as appropriate.
- Makes appropriate referrals to community resources.
- Performs other duties as assigned.
- Provides transition planning for hospice patients.
- Provides services in accordance with the mission, vision and values of Suburban Hospice
- Supervises social work students.
- Works closely with all members of the health care team in the development of the care plan based on the psychosocial assessment and accepted social work interventions.
- Courteous, friendly and willing to assist others.
- Ability to travel to all geographical areas serviced by the agency
- Master’s Degree in Social Work from Accredited school of Social Work or BSW with 2 years of health care experience.
- Must be a licensed clinical social worker with the Indiana Professional Licensing Agency
- Current holder of Indiana driver’s license and automobile liability insurance
