Use various assessment to determine the client’s capacity and decision-making abilities.

Provide education to the community and facilities about adult abuse, neglect, and exploitation.

Assess for safety to determine if a client is able to maintain living independently.

Implement in-home services to allow clients to remain in the community and maintain the highest level of safety.

Facilitate and Initiate Guardianship of incapacitated adults.

Partner with Law Enforcement and Regulatory Agencies in APS investigations.

Providing case management services that assist clients in becoming self-sufficient;

Qualifications, Special Certifications and Licenses

LICENSING, CERTIFICATIONS, and/or OTHER SPECIAL REQUIREMENTS:

Submitting case reports within agency requirements and meeting state-mandated deadlines;

Assisting with other agency responsibilities as required i.e., CPS and Foster Care assignments.

One year of related experience.

Documents assessments, investigations, safety plans, critical incident reports, detailed case contacts, progress notes, and service plans in a case management system.

Provides information and education to the community regarding area of assignment; makes presentations to schools and community organizations.

Assists with outreach and communication to the community; obtain speakers from educational and business fields.

Conducts initial assessment of child abuse and neglect reports and provides crisis counseling for reporters.

Investigates allegations of child abuse and neglect, which includes interviewing children, parents, and other involved parties.

Uses child welfare framework to perform comprehensive assessments of children and their families, including home visits to determine risk and safety.
