As a Home Health Quality Coordinator you need to know how to:

Perform quality assessment of clinical records including diagnosis coding, review of assessment and plans of care.

Collaborate with leadership to develop quality indicators and QAPI initiatives for agency.

Team up with associates to ensure that services are provided at quality levels consistent with professional standards and goals.

Quality/ Risk/Patient Safety/Just Culture/Project Management/Lean/IP certification.

Assists with quality management studies through data collection, data input and report development.

Analyzes risk management/patient safety principles and provides recommendations for the incorporation of risk management principles in each medical center.

Provides support services to appropriate medical center staff in implementing risk management principles into credentialing processes.

Must be able to work in a Labor/Management Partnership environment.

Minimum of three (3) years' experience working in quality operations in an acute care setting.

Medical diagnoses, procedures, and acceptable treatment patterns. Inpatient hospital experience preferred.

Training and educational development for adult learners.

Plan, perform, and evaluate educational opportunities through a variety of means.

Utilization review, quality, and peer review processes/procedures.

And ability to utilize personal computer and corresponding applications including Microsoft Office (Word, Excel, Access, PowerPoint).

Organizing workload and preparation of correspondence for provider community and physician consultant reviewers.

Quality/ Risk/Patient Safety/Just Culture/Project Management/Lean/IP certification.

Assists with quality management studies through data collection, data input and report development.

Analyzes risk management/patient safety principles and provides recommendations for the incorporation of risk management principles in each medical center.

Minimum of three (3) years experience working in quality operations in an acute care setting.

Acts as a consultant for quality, AR&L, risk, patient safety, and infection prevention activities and programs.

Collaborate with RN Educator to ensure AMAP training, certification and supervision of AMAP’s within assigned region; complete AMAP Certifications as needed

Assume basic responsibilities during RN vacancy to include report writing, incident reporting and attendance at meetings.

AHA or equivalent CPR certification.

Experience supervising large numbers of licensed and direct care staff.

Excellent written and verbal communication skills.

ESSENTIAL RESPONSIBILITIES (include But Are Not Limited To)

Recent experience as charge nurse or equivalent management skills preferred

Demonstrated skills in teaching, communication, problem-solving, and project development/management required

2 years experience as registered nurse, with a preference for candidates who have prior clinical experience in dermatology or oncology

Demonstrated clinical competence and advanced knowledge in area of practice require

One or more years experience in ambulatory care preferred

Knowledge in specialty area of practice

Review patient records for individual patient clinical issues related to transfusion.

Gather and review transfusion and transplant history to establish the patient transfusion profile.

Shifts Available: Days- 10 hours. 40 hours/week. 4/10s with one day off each week, no weekends, no holidays.

Hours/Days: 8 – 6:30; Monday – Friday.

Length of contract: 13 weeks
