Overview A Memory Care Coordinator is a professional who works with individuals who have memory-related conditions such as Alzheimer’s disease, dementia, and other forms of cognitive impairment. They provide support and assistance to individuals and their families in order to help them manage their condition and improve their quality of life. Memory Care Coordinators work in a variety of settings, including hospitals, nursing homes, assisted living facilities, and private practices. Detailed Job Description Memory Care Coordinators are responsible for providing support and assistance to individuals and their families who are living with memory-related conditions. They work with individuals to develop and implement individualized care plans that address their specific needs. Memory Care Coordinators also provide education and resources to individuals and their families about memory-related conditions, and they work with other healthcare professionals to coordinate care. Memory Care Coordinators may also provide counseling and support to individuals and their families, and they may facilitate support groups. Skills Required Memory Care Coordinators must have excellent communication and interpersonal skills in order to effectively interact with individuals and their families. They must also have strong organizational and problem-solving skills in order to develop and implement individualized care plans. Memory Care Coordinators must also be knowledgeable about memory-related conditions and resources available to individuals and their families. Qualifications Memory Care Coordinators typically need a bachelor’s degree in social work, psychology, or a related field.
