Years of Experience: Mid-Level. Minimum of three years-experience working with Seniors with at least one year being with persons with Dementia.

They work with individuals to develop and implement individualized care plans that address their specific needs. Memory Care Coordinators also provide education and resources to individuals and their families about memory-related conditions, and they work with other healthcare professionals to coordinate care. Memory Care Coordinators may also provide counseling and support to individuals and their families, and they may facilitate support groups. Skills Required Memory Care Coordinators must have excellent communication and interpersonal skills in order to effectively interact with individuals and their families.
