Maintains the client hospitalization log, including entering coordination notes, and sending electronic log to all office, field, and sales staff.

Schedules discharge visit / OASIS Collection or recert visit following case conference when task appears on action screen.

Schedules TIF OASIS collection visits and deletes remaining schedule.

Completes requested schedules for all add-ons and applicable orders:

Responds timely to system and/or clinician schedule requests to ensure referrals and ongoing care is scheduled per policy/orders and patient need.

Utilizes system scheduling module, communication tools and reports to ensure timely and well-coordinated patient visits effectively and accurately.

Utilizes other patient information systems to ensure complete referral information attached and available in EMR when appropriate.

Provides timely feedback to leadership when barriers to productivity, staffing needs and or untimely clinician response for communication and documentation.

Monitors assigned reports for compliance with requested scheduling and alerts when scheduling is outside of regulatory or agency policy.

Experience working in a managed care environment preferred

Assists health care providers, registered nurses and allied health professionals with examinations, treatments, special tests and routine procedures.

Obtains patient health history, laboratory specimens,vitalsigns and administers TB skin tests.

Reviews patient chart; records and reports observed symptoms, reactions, treatments, and changes in patient’s conditions.

Current state Certificate as a Medical Assistant

Associate’s degree or equivalent years of training and work experience

Minimum of 1 year of experience in healthcare, preferably Home Health

Excellent oral and written communication skills with clinical and non-clinical staff

Strong organizational skills, attention to detail and computer software proficiency

Monitors several portals for incoming referrals to process timely

Monitors emails and answers timely and efficiently

Answers incoming calls as needed

Maintains a current awareness of the skills and abilities or limitations of clinicians to ensure appropriate scheduling assignments

Manages and maintains staffing schedules according to client needs and location guidelines

Prior health care scheduling or staffing experience desired

One (1) year previous office work experience preferred

Greets and assists clients, employees, and the general public when entering the office providing excellent customer service

Coordinates orientation and training for new hires and active employees
