Conducts behavioral assessments to identify individual member knowledge skills, behavioral, social, financial, and other needs.
Diffuses situations with members who are resistant to health coaching and/or disease management interventions.
Coordinates with oncology provider/s to help manage overall cancer care and treatment plan.
Trouble-shoots patient difficulties, which include medications, appointments, services, finances, living and family situations.
Certification or advanced certification and/or license appropriate to field of specialty preferred.
Experience working with Oncology and Hospice Support.
Senior Health Educator - Part-Time
