Maintain and update all records and pertaining to the Family Market and School Pantry Programs specifically around grants management;

Facilitate the improvement of the program operational model with the DDP Manager, School-Based Programs;

Manage monthly reporting and client participation as required by Food Bank or any of its supporters;

Develop monthly menu and place orders within program specifications for Family Markets and School Pantry Programs;

Ensure monthly distribution calendar is accurate and communicate delivery details with the transportation team for all orders;

Nutrition Coordinator, Patient Food Service, Essential Duties And Responsibilities

Support and promote patient satisfaction and participate with a multidisciplinary team to improve quality care/services to patients.

Communicate problems or concerns with patients to appropriate personnel in a timely manner, following department procedures.

Qualifications For a Nutrition Coordinator

Coordinates projects within the department, including nutrition related in-services, interdepartmental nutrition education programs, and lectures.

Designs, develops and regularly updates nutrition education materials to meet the specialized needs of specific patient populations.

Participates in community focused education programs and external speaking events to promote service and education.

Certified Dietitian Nutritionist (CDN) from NYS Department of Education Office of the Professions, preferred, but not required.

Documents pertinent findings in the medical record as per policy.

Provides quality nutrition counseling to specific patient populations and family members as indicated.

The Nutrition Coordinator oversees the health and nutrition education services provided by all Local WIC Provider staff.

The Nutrition Coordinator reviews and approves all nutrition education materials (e.g., newsletters, recipes, bulletin boards, etc.)

Verification statement from a Dietetic Internship program accredited by the Accreditation Council for Education in Nutrition and Dietetics (ACEND); or

A minimum of one-year professional experience as a dietitian or nutritionist preferred.

The Nutrition Coordinator plans and implements the nutritional components of the Local WIC program.

The Nutrition Coordinator monitors and prepares reports, records and other data related to nutritional services.

Understanding of the demands of the MMA sport (i.e. physical, physiology, gender-specific, weight categories, weight management, travel requirements, etc.).

Coaches athletes through longitudinal weight management, weight classification strategies, and weight cutting for the sport of mixed martial arts.

Delivers individual and group nutrition educational sessions to world-class MMA fighters.

Engages with and supports continuous athlete education relating to anti-doping and USADA testing procedures.

Provides remote nutritional counselling through a variety of communication platforms to athletes situated around the world.

Within the rules of professional confidentiality, liaises with athletes, primary care managers, coaches and other support staff as appropriate.

Maintains hospital standards for a clean and quiet patient environment to maintain a positive patient care experience.

Maintains competency and knowledge of current standards of practice, trends, and developments in related scope of job role or practice.

Mentors staff and assists in education and competency check offs.

Provides a positive and professional representation of the organization.

Promotes culture of safety for patients and employees through proper identification, reporting, documentation, and prevention.

Adheres to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations.

Educate patients and their families on the importance of proper nutrition and dietary management in managing kidney disease.

Conduct initial nutrition assessments and follow-up evaluations for dialysis patients.

Develop and implement individualized nutrition plans for patients, taking into account their medical history, lab results, and current dietary habits.

Monitor and adjust patients' nutrition plans based on changes in their health status or treatment regimen.

Collaborate with the healthcare team, including physicians, nurses, and social workers, to provide comprehensive care for dialysis patients.

Monitor patients' nutritional status and provide recommendations for supplements or additional interventions as needed.
